Author's abstract

The correlation between sleep and ADHD is often discussed and controversial opinions exist. The main therapy is based on stimulant medication that can have a negative influence on sleep quality as well. Physical activity is assumed to be a positive candidate for an alternative treatment. Different types of exercise, measurement and target groups make it difficult to compare study results. To date, no consistent recommendations exist. A good sleep quality is necessary for all people and especially for children with ADHD, because they suffer from a deficit in attention and concentration, that can be worsened by tiredness. The purpose of the questionnaire study was to determine whether physical activity is associated with better sleep quality among children with ADHD diagnosis and normal controls and if there are differences between those two groups. In total 273 elementary school children and their parents were assessed with Children`s Sleep Habits Questionnaire (CSHQ), Sleep self report (SSR), ADHD rating scale (FBB-HKS) and leisure time questionnaires (CHILT, PAQ-C). Higher rates of sleep disturbances and evidence for higher rates of eveningness-types in children with ADHD were found. Furthermore, a small correlation of physical activity and sleep quality parameters, but only in the control group. It can be concluded, that sleep problems were more common in children with ADHD but an effect of exercise on sleep quality in this group could not be confirmed. The aim of the intervention study was to investigate the influence of different modes of exercise on sleep quality parameters in school-aged children with Attention Deficit Hyperactivity Disorder (ADHD) diagnosis. Polysomnographic (PSG) measurements were taken according to criteria of the American Academy of Sleep Medicine (AASM) on three nights following exercise session or control day, separated by one week and in random order. The study took part at the participants’ home environment and eleven children (nine boys / two girls, age 9.5 (SD 0.5), height 139.9 cm (SD 7.0), weight 31.9 kg (SD 7.7), BMI 16.11 (SD 3.14) with medical ADHD diagnosis participated in the study. An endurance session on a bicycle ergometer, a coordination exercise session on a wave board and the results on a day without physical activity were compared. Exercise intensity was measured by heart rate. The exercise sessions took part 3-4 h prior to bedtime and lasted 30 min. PSG measurement revealed no sleep complaints or sleep stage changes according to the different exercise conditions. REM-latency was increased after bicycle exercise and significant decreased after wave board exercise. Data suggests that physical exercise performed in the afternoon has an impact on REM-latency in ADHD children. Endurance exercise lengthens REM-latency so it is assumed to have a more positive influence on sleep patterns than coordination exercise. It can be hypothesized, that coordination exercise on a wave board had an exciting effect on the children, because a shortening in REM-latency can be involved in the processing of emotional or stressful situations. Furthermore, the effects of different exercise-modes and sleep quality on concentration performance and visual evoked potentials in children suffering from ADHD were explored. In the morning, different types of cognitive performance tests were performed, half an hour after awakening. Simultaneously event-related potentials were measured by EEG. ANOVA and Pearson correlation analysis revealed a significant effect between cognitive performance and both exercise and sleep quality. Furthermore, between event related potentials and sleep quality. Exercise as well as sleep quality are important factors influencing executive functions on the next morning in ADHD children.
