Einleitung: Eine Stressfraktur ist definiert als partielle oder komplette Fraktur eines Knochens durch rezidivierende Mikrotraumata. Stressfrakturen sind eine häufige Verletzung unter Athleten mit einer Inzidenz von 0,12 - 4,4 %. Lauf- und Sprungdisziplinen sind dabei die am häufigsten involvierten Sportarten. Stressfrakturen des medialen Malleolus sind sehr selten und gehören zu den Hochrisikostressfrakturen. Diagnosestellung und Therapie gestalten sich in der Praxis häufig problematisch, führen zu einer zeitlichen Verzögerung in der Therapie und sind daher eine anspruchsvolle Aufgabe des Sportmediziners. Therapeutisch wird zwischen konservativer und operativer Behandlung unterschieden. Methodischer Teil: Wir berichten von einem Sportler, der innerhalb von 21 Monaten eine Stressfraktur beider medialer Malleoli aufzeigte. Ergebnisse: Der Patient wurde links mit arthroskopischer Knorpelglättung und Anbohrung sowie Schraubenosteosynthese versorgt. Auf der rechten Seite erfolgte eine arthroskopische Knorpelglättung und Anbohrung ohne Osteosynthese. Beide Therapieregime führten zu einer Ausheilung. Diskussion: Basierend auf Literaturanalyse und Patientenbeispiel glauben wir, dass sowohl operative als auch konservative Therapie gute Ergebnisse erbringen kann, jedoch operativ eine schnellere Rückkehr zum Sport ermöglicht wird. Ein operativer Ansatz sollte beim entsprechenden Patientenkollektiv durchaus frühzeitig erwogen werden. Verf.-Referat

Introduction: A stress fracture can be defined as a partial or complete fracture of bone that results from repeated application of stress to the bone. Stress fractures are common injuries in athletes with an incidence of 0.12 - 4.4 %. Running and jumping are the most common sporting activities that have resulted in stress fractures. Stress fractures of the medial malleolus are extremely rare. Diagnosis and therapy are often difficult, result in a delayed begin of therapy and are a demanding task in sports medicine. Both surgical and conservative treatment are possible. Methods: We present the case of an athlete, who presented stress fractures of both medial malleolus in 21 months. Results: The patient was treated on one side with arthroscopy and an osteosynthesis and with arthroscopy on the other side. Each time the fracture healed without any problems. Discussion: Based on literature and our patient we believe that surgical and conservative treatment are possible options. However surgical procedures seem to bring back the athlete earlier to sporting activities. Surgery should be considered early in the right patients. Verf.-Referat
