Lacrosse is an action-packed sport in which players are subject to significant contact with other players, crosses, and the ball. As a result, players are required to wear basic protective devices such as helmets, facemasks, mouthpieces, gloves, shoulder pads, and arm pads. Goalkeepers, who must face oncoming players and shots on goal, must wear additional shin guards, chest protectors, and throat protectors; however, their hands are not afforded any more protection than a field player's hands. We have noted one particular type of injury sustained over a 24-month period in five thumbs of three goalkeepers of a National Collegiate Athletic Association Division I college lacrosse team. Two players sustained bilateral injuries, but in neither player were the injuries simultaneous. The player with unilateral involvement injured his dominant hand. Four injuries occurred to the hand at the top of the crosse shaft and one injury occurred to the hand at the base of the shaft. All five injuries were sustained from a direct impact of the ball against the tip of the thumb, transferring an axial load across the interphalangeal joint. The load resulted in a fracture of the distal phalanx at the interphalangeal joint in four of the injuries and at the proximal phalanx in one of the injuries. All five fractures resulted in acceptable outcomes. However, we believe that these injuries may be avoided with equipment better designed to protect the thumb, such as a molded plastic insert covering the tip and dorsum of the thumb or a custom-made glove with a more rigid thumb casing.
