Abstract des Autors
The concept of "biological" management of long bone shaft fractures has several features that have won it increasing favour in recent years: non-opening of the fracture site, no reaming, resumption of function as early as possible. The principles enunciated by Sarmiento many years ago therefore, are still valid. A total of 232 tibia fractures were treated at the Rizzoli Institute between September 1989 and September 1994 with a new device known as the "universal", "elastic" and "bundle" nail on the strength of its various characteristics. The series included 30 professional or high-level athletes (about 14%). The results were mostly very good. The athletes in particular reached union in less than 12 weeks on average. Early weight-bearing (with or without brace or cast protection) enabled these subjects to remain in top physical condition, and in some cases even resume competitive sport, during the treatment period.
