External fixation is a means of synthesis that absolutely complies with the biology of the fracture site, but involves iatrogenic damage at the points of entry of the holding components. It has two levels of indication in the treatment of leg fractures in athletes: - indications of necessity, emblematically shown by cortico-cancellous impaction with massive impairment of the skin and very unstable fractures, where the choice is virtually mandatory; - indications of opportunity, emblematically fractures that are biologically and mechanically sound, where it is remunerative whenever one can create assemblies with very low invasiveness, typically anterior or medial single side assemblies.
