The purpose of ther study was to evaluate the behavior of the lumbar spine of young athletes in the supine and standing position, taking into account all postural changes that the spine undergoes during physiological load, any alterations and any discrepancies in the detection of pathology between the two positions. Between August 2008 and October 2009, the authors evaluated 40 sportsman (20 men and 20 women) with symptoms of lumbar pain. All subjects underwent magnetic resonance imaging (MRI) with a dedicated scanner of 0.25 Tesla (G-Scan, Esaote Biomedica, Genoa, Italy) either in the conventional supine position, or in the weight-bearing position. Spin echo T1w and fast spin echo T2w images were acquired with a dedicated receiving coil for the lumbar spine (group B). Three radiologists, blinded on history and clinical examination of subjects, assessed, in consensus, variations of lumbar lordosis angle, of lumbo-sacral angle and presence of pathology on images acquired in both positions, in different sequences and in different planes. Conventional supine MRI was positive for pathology 30/40 while weight-bearing MRI was positive in 39/40 respectively. In particular, the evaluation in the weight-bearing position allowed in 9 cases to detect diseases that would have remained unrecognized if the evaluation had been performed only in the supine position. Compared to MRI in the supine position, weight-bearing MRI showed changes of the lumbo-sacral angle in 31/40 subjects and of lumbar lordosis angle in all 40 subjects. Imaging the lumbar spine in weight-bearing position with a dedicated MRI scanner and a dedicated coil might allow identification of pathology that may be overlooked if imaging patients only in the supine position. Verf.-Referat (geändert) Scopo del nostro lavoro è stato quello di valutare il comportamento della colonna lombare di giovani atleti in posizione supina ed ortostatica, valutando tutte le variazioni posturali a cui la colonna va incontro durante il carico fisiologico, le alterazione patologiche e le eventuali discrepanze nella rilevazione della patologia tra le due posizioni. Nel periodo compreso tra agosto 2008 e ottobre 2009 abbiamo valutato due gruppi di 40 atleti (20 uomini e 20 donne), composto da soggetti con sintomi di dolore lombare. Tutti i soggetti sono stati esaminati con risonanza magnetica (RM) tramite scanner dedicato da 0,25 Tesla (G-Scan, Esaote, Biomedica, Genova, Italia) sia in posizione convenzionale supina, sia in posizione sotto-carico. Sono state acquisite immagini spin-echo T1-pesate, e fast-spin echo T2-pesate, tramite una bobina ricevente dedicata per la colonna lombare. Tre radiologi, in cieco rispetto all’anamnesi e l’esame clinico dei pazienti, hanno valutato, in consenso, le variazioni dell’angolo di lordosi lombare, dell’angolo lombo-sacrale e la presenza di patologia sulle immagini acquisite in entrambe le posizioni, nelle differenti sequenze e piani di scansione. La RM convenzionale supina è risultata positiva per patologia in 30/40 pazienti, mentre la RM sotto-carico è risultata positiva 39/40 pazienti rispettivamente. In particolare, la valutazione in posizione ortostatica ha permesso, in nove casi, di riscontrare patologie che sarebbero rimaste misconosciute se la valutazione fosse stata eseguita con la sola posizione supina. Rispetto alla RM in posizione supina, la RM in ortostatismo ha dimostrato modificazioni dell’angolo lombo-sacrale in 31/40 soggetti e dell’angolo di lordosi lombare in tutti e 40 i soggetti. La diagnostica per immagini della colonna lombare in posizione ortostatica con una RM dedicata e bobine dedicate può permettere l’identificazione di alterazioni patologiche che potrebbero rimanere misconosciute se i pazienti venissero esaminati nella sola posizione supina.
