Abstract
Evaluation of the literature on the anatomy, aetiology, diagnosis and treatment of Sinus Tarsi Syndrome (STS) based on the best available literature. Literature review of PubMed, CINAHL, Web of Science and reference lists and assessment using the CBO classification system. 64 articles were included. STS is a clinical diagnosis. A useable definition is: pain in the sinus tarsi which worsens on palpation, supination and adduction and is improved by rest, sometimes a feeling of hind foot instability, and improvement after injection with local anaesthetic. The aetiology is likely to be due to various posttraumatic abnormalities like ligament, tendon, nerve, synovial and/or vene laesions in the tarsal sinus. There is level B evidence that additional investigations such as, arthrography, EMG, MRI and arthroscopy have a low sensitivity and specificity. There is level C evidence that therapy with multiple local anaesthetic and corticosteroid injections or an operation gives good results. Verf.-Referat
Abstract
Evaluatie van anatomie, etiologie, diagnostiek en behandeling van het sinus tarsi-syndroom (STS) op basis van het best beschikbare bewijs uit de literatuur. Literatuurreview van PubMed, CINAHL, Web of Science en referentielijsten en beoordeeling volgens het CBO-classificatiesysteem. 64 artikelen werden geincludeerd, STS is een klinische diagnose. Een bruikbare definitie van het STS is: pijn over de sinus tarsi toenemend door palpatie, supinatie en adductie en afnemend door rust, met eventueel een subjectief gevoel van instabiliteit van de achtervoet waarbij injectie met een lokaal anestheticum verbetering geeft. Etiologisch lijkt er sprake te zijn van verschillende vormen van posttraumatische afwijkingen van de weefselstructuren in de sinus tarsi. Aanvullende onderzoeken als arthrografie, EMG-onderzoek, MRI en diagnostische arthroscopie hebben een lage sensitiviteit en specificiteit (niveau B-bewijs). Er is niveau C-bewijs dat behandeling met multipele injecties van een lokaal anestheticum, corticosteroiden of een operatie goede resultaten geeft. Verf.-Referat
