Abstract

Infrapatellar nerve neuropathy is an entrapment neuropathy of the infrapatellar branch of the saphenous nerve. This can result in pain and altered sensibility on the medial anterior side of the knee. Often patients have had symptoms for a long time before the diagnosis is made. Improved awareness of this problem among health professionals is important to accelerate the process of diagnosis and treatment. In this article two case reports will be described: one posttraumatic neuropathy and one idiopathic neuropathy. An infiltration with a local anaesthetic can be used as a diagnostic tool to test if the pain disappears completely for a couple of hours. If this test is positive, one can consider a surgical procedure: neurolysis or conservative treatment. Because there is a lack of knowledge about the syndrome, further research on long term effects and alternative treatment options is recommended. Verf.-Referat

Abstract

Nervus infrapatellaris neuropathie is een entrapment neuropathie van de infrapatellaire zenuwtak van de nervus saphenus. Dit kan pijnklachten met sensibiliteitstoornissen veroorzaken aan de mediale anteriore zijde van de knie. De diagnose wordt veelal pas laat gesteld. Meer kennis over het ziektebeeld onder lparalrnedici is van belang om het proces van diagnose en behandeling te ersnellen. In deze klinische les worden twee casus beschreven van zowel een posttraumatische als een idiopathische neuropathie. Als diagnosticum kan een proefinfiltratie met een lokaal anaestheticum gegeven worden om te testen of de klachten gedurende een paar uur volledig wegblijven. Is deze test positief, dan kan overgegaan worden tot een operatieve ingreep: neurolyse of conservatieve behandeling. Daar er nog veel onbekend is over het ziektebeeld, verdient na der onderzoek naar alternatieve behandelingsopties en effecten op langere termijn aanbeveling. Verf.-Referat
