Unfortunately, a basic problem arises in the process of therapy: to optimize individually the demands in order to reach the existing abilities or to optimize performance and reduce risks of injury at the same time. The aim of the study was the development of a movement- and sportspecific control- and test instrument, which is based on an integrative approach, considering the load-effect situation which enables a goal oriented therapy regulation. The results presented here are based on a single-case study of a person with kidney transplantation. We designed a step-test for breast stroke with load and relief of strain in a swimming flume and investigate physiological and psychological indicators as well as indicators of the movement - co-ordination. Finally we could find a secured load-rate to optimize the therapy-success and to minimize load-connected risks.
