Osteoarthritic knee rehabilitation in an elderly male: a functional approach

Abstract des Autors

Objective: To discuss the use of a multifaceted-functional approach in the rehabilitation of a patient suffering from unilateral knee osteoarthritis. Clinical features: A 68-year-old caucasian male presented with a seven year history of left knee pain and inabilities to perform daily activities. Radiographic imaging revealed degenerative joint disease and osteophytic formations throughout the left knee joint. Gait dysfunction, muscle atrophy and weakness and severe visible varus deformities of the left knee joint were also present.

Intervention and outcome: The patient was first diagnosed in 1996 with left knee osteoarthritis and prescribed 400 mg of Sulindac per day, and an orthopedic unloading lateral stabilization brace. As of October 9, 1997, the patient was still using medication and bracing along with performing non-weight bearing proprioception and knee strengthening exercises prescribed by a physician three months prior to this date. The patient reported minimal changes in symptoms and function since the initiation of the medication, bracing, and exercises. A multifaceted-functional rehabilitation program was initiated on October 9, 1997, and included components in flexibility, aerobic capacity, muscle strengthening, joint proprioception, and gait retraining. The program was carried on for three months at which time the patient had exhibited significantly improved function and was released to home care. Conclusions: The use of a multifaceted-functional approach in rehabilitating osteoarthritic knee patients seemed essential in controlling both the progression and severity of the arthritic disease process in this patient.
