Abstract des Autors

The central role of insulin resistance in the pathogenesis of atheroma and type-2 diabetes is generally accepted. In the prospective TUeFF (Tuebinger Familien Frueherkennung) project we are screening for insulin resistance in healthy young individuals with a family history of type-2 diabetes. In 103 subjects (45 male, 58 female; average age 33 years) with both normal casual blood pressure (140/90 mmHg) and normal oral glucose tolerance (oGTT), insulin sensitivity was assessed by isoglycaemic hyperinsulinaemic glucose clamp. Physical fitness was determined by spiroergometry (VO2max), habitual physical activity by a standardized questionnaire. 61 individuals were classified as insulin resistant (glucose metabolic clearance rate (MCR) <7 ml/kg/min), 7 as borderline and 35 as insulin sensitive (MCR >8ml/kg/min). Resistant individuals had higher BMI, percentage of body fat, insulin levels (AUC) and blood glucose levels (AUC) in the oGTT and showed a lower level of physical fitness than the comparable sensitive individuals. In young healthy offspring of type-2-diabetic patients, symptoms of the insulin-resistance-syndrome are frequently accompanied by lower physical fitness with respect to HPA. The question remains, whether this is cause or consequence of insulin resistance. Verf.-Referat
