Patellofemoral pain is a common complaint of patients seeking chiropractic care. It is essential to understand how to perform a detailed analysis of the condition to design an effective treatment and rehabilitation protocol. Current theories for the etiology of patellofemoral pain suggest stimulation of superficial bone nerve endings, irritation of lateral retinacular supports, or entrapment of the synovial fringe. This article discusses orthopedic and radiological assessment for this condition and examines management considerations. More expensive kinematic magnetic resonance imaging studies of the patellofemoral joint may be of greater value in providing diagnostic information. Treatment protocols include acute phase care, correction of faulty biomechanics, and strength, flexibility, and proprioceptive training of appropriate musculature. Manipulation of the spine, pelvis, and knee has been advocated in the chiropractic literature and is used extensively by practicing DCs; however, as to its effectiveness, there has been little published research.
