Abstract des Autors

Sport-induced hypoglycemia in type I diabetes requires a reduction of the insulin dose or an increase of the carbohydrate intake respectively. 11 type I diabetics, (27-57 years) performed a 1-week bicycle tour in the Netherlands, covering a distance of 63 km/km in about 5-6 h. During cycling, their heart rate varied between 110 and 130 beats/min. The participants were advised to reduce their insulin dose and to increase their carbohydrate intake according to self monitored blood glucose concentrations. The intake of carbohydrates increased by 37+/-41% in the time between 6 am and 6 pm and by 22+/-36% during the rest of the day. Participants on intensive insulin therapy (Pen/CSII) reduced their prandial insulin bolus on average by 50%. The basal insulin dose was reduced by 21+/-6% in the morning and by 18+/-11% in the evening. Total reduction of insulin amounted to 27+/-9%, taking into consideration the additional carbohydrate intake. In physical activities of similar intensity and duration with the same carbohydrate intake, the insulin dose for type I diabetics can be reduced by 30%. Verf.-Referat
