Abstract des Autors

Many experiences underline that systolic blood pressure values at the top and after a physical exercise (SBP-rate) achieved a clinical relevance in order of the diagnosis and prognosis of ischemic cardiomyopathie. We studied 40 male patients (36.8+/-12.4 years old): 20 normotensives and 20 with a diagnosis of hypertension (office blood pressure: SBP: 158.3+/-12.4 mmHg; DBP: 94.4+/-6.8 mmHg). All patients performed: a) bidimensional echocardiography (8 hypertensives showed left ventricular hypertrophy-LVH (interventricular septum = 12+/-1.8 mm)); b) Holter ECG; c) Ergo-metrics test (by Ergo-Metrics 900 Computer Ergometer). During this test, blood pressure was recorded at baseline, at the end and, after exercise, in the recovery time. The data obtained show higher SBP-rate values, in each recovery phase, in hypertensives than in normotensives. Hypertensives with LVH, furthermore, have slower recovery times and peculiar SBP-rate, than hypertensives without LVH. This hemodynamic profile seems to be related to the increased vascular resistances. In previous studies, other authors found a similar responsiveness pattern at the exercise test in patients with ischemic cardiomyopathy, which could be characterized by the same autonomic disreactivity of hypertensive state.
