The Medical Commission (MC) of the International Olympic Committee (IOC) has banned the group of diuretics for the Olympic Winter and Summer Games 1988. Diuretics are misused first in sports with weight classes to reduce body weight in order to qualify for a lower weight class and second to avoid a positive result. Diuretics may reduce the concentration of the excreted dope agent to such an extend that the resulting concentration remains under the detection limit of the applied analytical methods. In general, not all urine samples must be tested for diuretics. The Medical Commission (MC) of the IOC suggests to analyse an urine only when the urine density is equal or lower than 1.010 g/ml and when the pH-value is equal or higher than 7.0. The screening of diuretics is performed by HPLC with UV-detection. Two extraction procedures are common: 1. Extraction of neutral and acidic diuretics from acidified urine (pH=2.0) with diethylether; 2. Extraction of basic diuretics from alkalized urine (pH=12) with diethylether. The subsequent HPLC analysis is performed by use of reversed-phase columns RP-18 and solvent systems with acetonitrile/water/phosphoric acid for acidic and neutral diuretics and acetonitrile/water/ammonium/acetate/triethylamine for basic diuretics. The diuretics are detected with diode array detector (DAD) at different substance specific wavelengths. In case of a positive screening result the sample is methylated and analysed by gaschromatography with mass-spectrometric detection (GC/MSD).
