Bei 34.742 Sportverletzungen (1972-1997) werden 701 (2,0%) Achillessehnenrupturen beobachtet. Insgesamt wurden bis 1/2000 911 Faelle behandelt, davon 791 retrospektiv, 73 prospektiv analysiert. Risikoreiche Sportarten sind Faustball (14,6%), Badminton (11,7%), Gymnastik, Squash und Tennis. Maenner sind in 86% betroffen. In 96% handelt es sich um indirekte Verletzungen, insbesondere beim Antritt (33,6%), Sturz und Verdrehtrauma (24,8%), Sprint oder Lauf (15,8%). Die Diagnose wird vielfach primaer nicht gestellt - sicherste klinische Zeichen sind Dellenbildung (100%), Unfaehigkeit des Einbeinzehenstands (97%) waehrend Thompson-Test (67%) und Flexion gegen Widerstand (64%) unsicherer sind. Apparativ ist roentgenologisch das Kargersche Dreieck nur in 40% positiv, waehrend vor allem die Ultraschall-Sonographie in 91% oder in besonderen Faellen das MRT mit 99% valent sind. Die Therapie war in 97% operativ - in den letzten Jahren zunehmend minimalinvasiv in perkutaner Technik (57,6%), End-zu-Endnaehte folgten in 19%, kombiniert mit plantaris Durchflechtung in 11%. Die Rehabilitation erfolgt fruehfuenktionell im Adipromed-Vario-Stabil-Schuh. Ein Behandlungsalgorithmus wird vorgestellt. Verf.-Referat
Abstract
Among 34742 sports injuries (1972-1997), 701 (2%) Achilles tendon
ruptures were observed. Overall, up to 1/2000, 911 cases were treated,
with 791 of them analyzed retrospectively and 73 prospectively. Highrisk
types of sports are „faustball“ (14.6%), badminton (11.7%), gymnastics,
Squash and tennis. Men account for 86% of those affected. In 96%,
injuries are indirekt, especially in stepping (33.6%), falls and twists
(24.8%) sprint or running (15.8%). The diagnosis is often not made primarily
– the most certain clinical signs are indentions (100%). Incapability
of standing on the toes with only one foot (97%) during the
Thompson test (67%) and flexion against resistance (64%) are less certain.
In x-rays, the Karger triangle is positive in only 40%, while especially
ultrasound sonography in 91% and in special cases MRT are valent
to 99%.
Therapy was surgical in 97% – in recent years increasingly using minimally
invasive percutaneous techniques (57.6%). End to end sutures were
made in 19%, combined with plantaris meshing in 11%. Rehabilitation
was initiated with combined early functional means, usually in Adipromed-
