This article presents the evolution and treatment of a pathology of the myotendinous joint and long peroneal muscle caused by overuse, in a highly professional athlete, presenting an anomalous isolated compromission of the functional unit of long peroneal muscle that, despite the conservative treatment and the cures of the medical team, presented a progressive negative evolution, so much that it required surgery. Presentiamo l’evoluzione ed il trattamento di una patologia da overuse del muscolo, della giunzione miotendinea e del tendine del peroneo lungo in un atleta di alto livello con anomala compromissione isolata dell’unità funzionale del peroneo lungo che, nonostante il trattamento conservativo e le cure del team medico, ha presentato una progressiva evoluzione peggiorativa, tanto da richiedere un tempo chirurgico.
