Abstract

Extracorporeal shockwave therapy is increasingly used to treat tendinopathy. Some think of extracorporeal shockwave therapy as being controversial as a treatment. Reasons for this are varying results of treatment in clinical studies, a great diversity in protocols and shockwave generators and lack of knowledge about the exact working mechanisms. The aim of this article is to describe extracorporeal shockwave therapy as a treatment option for tendinopathy. First a description of shockwaves is given. Then the generation of shockwaves, working mechanisms and important treatment parameters are described. Special attention is given to the differences between focused shockwaves and radial pressure waves. Although there is a large body of research into extracorporeal shockwave therapy, more research is needed to settle the controversy surrounding this therapy. Verf.-Referat

Abstract

Extracorporale shockwave therapie wordt in toenemende mate gebruikt voor het behandelen van tendinopathieen. Extracorporale shockwave therapie wordt door sommigen gezien als een controversiele behandelmethode. Redenen hiervoor zijn wisselende resultaten in klinische studies, een grote hoeveelheid aan protocollen en shockwave-generatoren en de onbekendheid van het exacte werkingsmechanisme. Het doel van dit artikel is om een beschrijving te geven van extracorporale shockwave therapie als behandelmethode voor tendinopathie. Achtereenvolgens wordt beschreven wat shockwaves zijn, hoe ze worden opgewekt, wat de voorgestelde werkingmechanismen zijn en wat mogelijke parameters zijn bij het behandelen. Speciale aandacht wordt besteed aan de verschillen tussen gefocusseerde shockwaves en radiale drukgolven. Hoewel er al veel onderzoek is gedaan naar extracorporale shockwave therapie is er meer onderzoek nodig voor een beter begrip van de behandelmethode om zo de controverse rond deze behandelmethode te verminderen.
