Abstract

Um die Durchführbarkeit und den Einfluss von bewegungstherapeutischen Maßnahmen schon während der stationären Phase einer Stammzelltransplantation zu überprüfen und den Einfluss auf die psycho-soziale Befindlichkeit der Patienten zu untersuchen, wurde eine randomisierte kontrollierte Studie in der Klinik für Knochenmarktransplantation in Idar-Oberstein erhoben. 49 Patienten wurden einer Trainingsgruppe und einer Kontrollgruppe randomisiert zugeordnet. Das Programm der Trainingsgruppe (TG) setzte sich aus aerobem Ausdauertraining und bewegungstherapeutischem Alltagstraining zusammen, während die Kontrollgruppe (KG) einmal pro Tag die in der Klinik übliche Krankengymnastik verordnet bekam. Zur Evaluation der psycho-sozialen Befindlichkeit wurde der Basler Befindlichkeitsfragebogen zunächst zweimal pro Woche und ab Tag + 8 (nach der Transplantation) einmal pro Woche bis zur Entlassung ausgeteilt. Die Untersuchungen wiesen eine komplikationslose Durchführbarkeit des bewegungstherapeutischen Trainingsprogramms auf. Die Befindlichkeit zeigte in der TG von der stationären Aufnahme bis zur Entlassung im Vergleich zur KG in vier von fünf Befindlichkeitsfaktoren signifikant (Intrapsychischer Gleichgewichtszustand, p = 0,05; Vitalität, p = 0,011) und hochsignifikant (Vigilität, p = 0,002; Summenscore, p = 0,005) bessere Werte. Zusammenfassend belegt die vorliegende Untersuchung die mehrheitlich positiven Effekte bewegungstherapeutischer Aktivitäten auf die (momentane) Befindlichkeit des Patienten über die gesamte Phase einer Stammzelltransplantation, die jedoch in weiteren Studien bestätigt werden müssen. Abstract

In order to study and test the practicability and influence of physiotherapy on the psycho-social condition of patients during a stationary phase following marrow transplantation, a randomised controlled study was carried out in the Clinic for Marrow Transplantation in Idar-Oberstein. This led to a randomised assignment of 49 patients in two groups: Training Group (TG) and Control Group (CG). The TG programme comprised of daily aerobic exercise and physiotherapy whereas the CG programme, carried out once daily, encompassed the usual remedial gymnastics in the clinic. In evaluating the psycho-social status, the Basler Health Status questionnaire was used: initially twice a week and then once weekly after the eighth day after of the transplantation until discharge. The results of the study indicate an unproblematic implementation of the physical therapy programme. Regarding the influence of the programme on health status, the results show that TG (from admission to discharge) has significantly better parameters: i. e. significant in four out of five factors regarding health status (intra-physical equilibrium p = 0.05; vitality p = 0.011) and highly significant (vigilance p = 0.002; total score p = 0.005). All in all, this study shows several positive effects of physiotherapy on the momentary health status of the patients during the entire phase of marrow transplantation.
