Abstract
Die geschlossene Ringbandruptur bei Sportkletterern stellt seit etwa 20 Jahren ein neuartiges Verletzungsmuster dar. Bei 50 dokumentierten Fällen mit Ringbandrupturen handelt es sich zu 90% um singuläre, um 10% um multiple Ringbandrupturen eines Langfingers. Nach klinischer Verdachtsdiagnose und Ausschluss einer knöchernen Verletzung gibt die Sonografie Aufschluss über Ringbandzerrung, singuläre Ringbandruptur oder Mehrfachruptur. Ist mittels Sonografie keine eindeutige Diagnosestellung möglich, gibt die MRT weiteren Aufschluss. Bei der singulären Ringbandruptur erfolgt ein konservatives Vorgehen mit Ruhigstellung und frühfunktioneller Behandlung. Bei der Mehrfachverletzung sollte die operative Rekonstruktion angestrebt werden. Verf.-Referat
Abstract
The closed traumatic rupture of a flexor tendon pulley of sportclimbers appeared as a new complex finger trauma in the mid eigthies. By fifty documented cases of pulley ruptures, we had 90% singular and 10% multiple pulley ruptures. Leading is the ring finger, second the middle finger. After clinical suspicion and exclusion of fracture or injury to the capsula/ligamentsystem, the diagnosis can be proven with ultrasound examination. If this fails to give exact diagnosis an MRT should be performed. For a single rupture, we recommend conservative procedure, with initial immobilisation and early functional treatment. Verf.-Referat
