abstract	Das "tennis leg" ist eine typische Muskelverletzung von Maennern ueber 40 Jahren und tritt bevorzugt beim Racketsport, Skifahren und Laufen auf. Es handelt sich um eine inkomplette bis vollstaendige Ruptur des medialen Bauchs des M. gastrocnemius, die in der Regel sonographisch gut dokumentierbar ist. Leitsymptome sind ploetzliche messerstichartige Beschwerden, die zu einer obligaten Sportunterbrechung fuehren. Klinisch finden sich lokale Druck- und Dehnungsschmerzhaftigkeit sowie eine Dellenbildung. 25 Patienten wurden entsprechend einer neuen Gradeinteilung prospektiv sonographisch untersucht und der weitere Verlauf festgehalten. Die dem Schweregrad angepasste konsequente konservative Therapie fuehrte nach etwa zwei Wochen zu einer weitestgehenden Schmerzfreiheit wobei die sportliche Vollbelastbarkeit abhaengig von der Disziplin fruehestens nach drei Wochen wieder gegeben war. Die Sonographie ist in der Lage, Ausmass und Schweregrad der Laesion aufzuzeigen und somit einen gezielten Einsatz physiotherapeutischer Anwendungsformen zu ermoeglichen. Darueberhinaus laesst sie prognostische Schluesse zu. Verf.-Referat Tennis leg is a common muscle injury in men older than 40 years participating in raquet sports, alpine skiing and running. It is defined as a partial or total rupture of the medial head of the gastrocnemius muscle which usually is well demonstrable by ultrasound. Main symptom is a sudden sharp pain leading to an obligate immediate stop of sport activity. Clinically pain on local pressure or stretching of the muscle will be reported, local defects can be palpated. 25 patients were prospectively examined by ultrasound according to a new developed grading system. A grade-adapted conservative therapy program led to a wide relief of pain after about two weeks. Full range of sport ability was only regained after at least three weeks. Ultrasonography is able to clearly demonstrate the size and grade of the lesion, thus enabling a welldefined therapy program and prognostic conclusions.
