We report a series of 25 cases of surgical repair after severe lesions of the metacarpophalangeal volar plate ligaments. Such lesions usually occur in athletes and result from trauma-induced hyperextension of the metacarpophalangeal joints. The structure of the volar plate, the main static element stabilizing hyperextension, is reviewed together with the dynamic elements including the medial thenar and lateral thenar muscles which terminate on the sesamoid bones at the base of the first phalanx. In this series, an anterior access was used for surgical repair. The metacarpophalangeal pulley was opened for direct resinsertion of the volar plate and reconstruction plasty of the pulley using the superficial part of the terminal tendon of the adductor pollicis.
