Abstract

In addition to establishing the precise diagnosis and location of Achilles tendon ruptures, ultrasonography permits their fine differentiation and both quantitative and qualitative assessment of their treatment and healing. Twenty patients with acute rupture were examined in the course of a randomised, prospective and as yet uncompleted trial of an operative-functional versus conservative-functional management, using a newly developed boot. Adaptation of the tendon stumps in plantar flexion was observed in 17/24 patients. Continuity of the tendon was restored in all cases after 2 weeks. A remarkable increase in tendon regeneration at the rupture vs the suture site was documented between the 4th and the 12th week. An assessment is made of variations in tendon structure and a four-degree classification is presented. Verf.-Referat
