Ventricular repolarization disturbances and hypertrophy in athletes

Abstract
The relationship between ventricular repolarization disturbances (VRD) and cardiac hypertrophy in athletes is not well established. VRD are reported in an incidental way in well trained athletes, but certainly more frequently in subjects with hypertrophic cardiomyopathy (HCM). 23 athletes (9 cycling, 6 rowing, 3 canoeing, 2 basketball, 1 bodybuilding, 1 shot putting, 1 volleyball) chosen from a group of 271 top level olympic athletes in the basis of the incidental discovery of end-diastolic interventricular septum thickness more than 15 mm and 6 subjects with non obstructive HCM at rest were examined by mono and bidimensional echocardiography. VRD (inverted T-waves) were observed in resting ECG in 3 of the 23 athletes of the 6 with HCM. Echocardiographic data significantly differed in the two groups. Compared with athletes (physiological hypertrophy) HCM subjects (pathological hypertrophy) had: - reduced end-diastolic diameter; - non homogeneous and usually asymmetric hypertrophy which does not regress even after a long period of detraining; - systolic obliteration of the left ventricular cavity; - in some cases, presence of obstruction after provocative tests.
