Abstract des Autors
Die akute Verletzung des Schultereckgelenkes (Acromioclavicular- oder AC-Gelenk) gehört aufgrund ihrer Häufigkeit zum klinischen Behandlungsalltag. Eine gute Kenntnis und ein Verständnis der funktionellen Anatomie ist für die adäquate Behandlung von AC-Gelenksverletzungen notwendig. Typ 1- und 2-Verletzungen nach Rockwood sollten konservativ behandelt werden. Grad 4 und höher sollten operativ versorgt werden. Für Instabilitäten vom Typ 3 nach Rockwood kann auf Basis der momentan vorliegenden Literatur weiterhin keine klare Empfehlung für ein operatives oder konservatives Verfahren gegeben werden. Bei der operativen Versorgung höhergradiger Instabilitäten finden die Hakenplatte als offenes Verfahren und zunehmend auch mini-open oder arthroskopisch-assistierte flexible Faden-Plättchen-Techniken Anwendung. Im vorliegenden Beitrag werden ausgewählte Aspekte zur Behandlung der Instabilität des AC-Gelenkes beleuchtet.
Abstract des Autors
Acute injuries of the acromioclavicular joint (AC joint) are frequently observed and are a part our daily practise. An adequate knowledge and understanding of functional anatomy is necessary for successful treatment of AC joint injuries. Type 1 and 2 injuries (according to Rockwood) should be treated conservatively. Grade 4 and higher should be treated surgically. For type 3 instability according to Rockwood, no clear recommendations for an operative or conservative method can be given on the basis of the currently available literature. For the surgical treatment of high-grade instabilities, the hook plate is commonly used as an open procedure and, increasingly, also mini-open or arthroscopic-assisted flexible button-systems are used. In this article, selected aspects for treating the instability of the AC joint are discussed.
