Die Messung der Blutlaktatkonzentration bei körperlicher Belastung ist weiterhin eines der typischen und wichtigsten diagnostischen Verfahren innerhalb der internistisch orientierten Sportmedizin und Belastungsphysiologie. Die hier vorliegende Übersicht aktualisiert die wesentlichen methodischen Aspekte und diagnostischen Interpretationsmöglichkeiten der Laktatmessung bei körperlicher Beanspruchung. Haupteinsatzbereich der Laktatmessung ist dabei die Ausdauerleislungsdiagnostik und die sportmedizinische Trainingssteuerung von Sportlern oder Patienten, manchmal ergänzt durch Spiroergometrie. Seltener erfolgt die sportmedizinisch-klinische Ausschlussdiagnostik metabolischer Myopathien durch Messung der Laktatkonzentration. Für eine erfolgreiche Durchführung der Methode müssen potenzielle Artefaktquellen wie z.B. fehlerhafte Abnahmebedingungen minimiert werden. Eine Kenntnis der Prinzipien kompartimentaler Verteilung von Laktat ist wiederum zur Auswahl von Belastungsprotokollen und der abschließenden Interpretation hinsichtlich der so genannten anaeroben Schwellen sinnvoll. Measurement and interpretation of increasing blood lactate concentrations during exercise is a typical and important diagnostic tool within exercise-oriented sports medicine and exercise physiology. This review article updates the main methodological aspects and diagnostic-specific interpretations of lactate measurements during standardized exercise tests. Thereby, main application of lactate measurements is the endurance performance diagnostics and sports medicine training control of athletes or patients, while sometimes accompanied by spiroergometry. Less often in sports medicine, clinical diagnosis of metabolic myopathies by measuring blood lactate concentration is applied. For a successful implementation of the method, potential artifact sources such as faulty or non-standardized blood-sampling conditions must be ruled out. The principles of compartmental distribution of lactate are relevant for the justification and selection of exercise protocols and the final interpretation in terms of "anaerobic thresholds".
