Abstract

This study evaluated the incidence of injuries in adult elite fencers (over age 18 years). An anonymous questionnaire was distributed to the 432 adult fencers taking part in the second national qualification tournament (Foggia, 18-20 April 2008). The questionnaire contained items on sex, age, height, body weight, weapon, weapon arm, weekly practice (hours, sessions, bouts), type and duration of warm-up, competition level, number, location and type of injury sustained in the last three years and whether the injury occurred during practice or competition. Of the 432 fencers who received the questionnaire, 218 (50.5 percent) responded; six questionnaires were discarded because incomplete or incorectly compiled. Of the 212 athletes whose questionnaires were used in the final analysis, 122 were males and 90 females; 138 were epée, 53 foil, and 20 sabre fencers; 52 (25 percent) were left-handed (40 males and 12 females), with a higher proportion of left-handed male than female fencers (32.8 percent versus 13.3 percent; P=0.002). International level fencers (N.=78) practiced more than national level fencers (N.=129) in the number of sessions (4.7 +/- 1.4 versus 3.5 +/- 1.5; P<0.001) and weekly hours (12h24min +/- 6h13min versus 7h49min +/- 4h39min; P<0.001). The total number of injuries was 202, with a mean annual risk of injury of 31.8 percent. The most common injuries were thigh muscle strains (28.2 percent); ankle sprains (14.9 percent), knee tendinitis (7.4 percent), and Achilles tendinitis (5 percent). Occurrence of injury was associated with competitive level (P=0.010), with a higher incidence among those at the international level, and with weapon arm: the mean annual risk of injury was 40.4 percent in left-handed and 29.0 percent in right-handed fencers (P=0.026). The higher incidence of injuries among the left-handed fencers may be because they compete against an opponent using the opposing hand. Further study is needed to establish which aspects (technical, tactical, postural or others) may be related to greater use of musculoskeletal structures in left-handed fencers and related increased risk of injury. Lo scopo del presente studio è stato quello di valutare l’incidenza d’infortuni negli schermitori d’alto livello tecnico di età superiore ai 18 anni. Ciascuno dei 432 atleti italiani maggiorenni che hanno preso parte alla 2° prova di qualificazione nazionale di scherma (Foggia, 18-20 aprile 2008) è stato invitato a compilare un questionario anonimo. Esso riguardava sesso, età, altezza, massa corporea, arma praticata, braccio armato, volume settimanale d’allenamento (ore, sedute, assalti), tipologia e durata del riscaldamento, livello agonistico, numero, localizzazione e tipo degli infortuni negli ultimi tre anni, e loro occorrenza in allenamento o competizione. Duecentodiciotto atleti (50,5 per cento) hanno restituito il questionario; sei questionari sono stati scartati perché incompleti o erroneamente redatti. I 212 atleti studiati (122 di sesso maschile e 90 di sesso femminile) erano rappresentati da 138 spadisti, 53 fiorettisti e 20 sciabolatori. I mancini erano 52 (25 per cento, 40 di sesso maschile e 12 di sesso femminile): la proporzione di mancini era maggiore nei maschi (32,8 per cento) rispetto alle atlete (13,3 per cento) (P=0,002). Gli atleti di livello internazionale (N.=78) si allenavano di più rispetto a quelli di livello nazionale (N.=129) per numero di sedute (4,7 +/- 1,4 vs. 3,5 +/- 1,5; P<0,001) e ore settimanali (12h24’ +/- 6h13’ vs. 7h49’ +/- 4h39’; P<0,001). Il numero totale d’infortuni è stato di 202, con un rischio medio annuale d’infortunio pari al 31,8 per cento. Gli infortuni più frequenti erano rappresentati da distrazioni muscolari della coscia (28,2 per cento), distorsioni di caviglia (14,9 per cento), tendiniti rotulee (7,4 per cento) e achillee (5 per cento). L’occorrenza d’infortuni era associata al livello agonistico (P=0,010), con incidenza maggiore negli atleti di livello internazionale, e al braccio armato: il rischio medio annuale d’infortunio era pari al 40,4 per cento nei mancini e al 29 per cento nei destri (P=0,026). La maggior incidenza d’infortuni osservata nei mancini potrebbe essere legata al fatto che essi devono combattere con un atleta di mano opposta. Resta da stabilire quali aspetti (tecnici, tattici, posturali o altri) potrebbero essere legati a un maggiore impegno delle strutture muscolari e osteoarticolari dei mancini con conseguente aumento degli infortuni.
