Abstract des Autors

Die Unterschenkelfraktur ist eine häufige und schwer wiegende Verletzung im alpinen Skisport. Im Hochleistungssport galt diese Verletzung über lange Zeit als wahrscheinliches Karriereende. Es liegen bisher keine Angaben über Behandlungskonzepte und mögliche Komplikationen bei professionellen Skisportlern mit Unterschenkelfrakturen vor. Wir stellen drei Kasuistiken über alpine Skiprofis, welche Unterschenkelfrakturen bei Ausübung ihrer Sportart erlitten haben, vor. Die Frakturformen und die Therapieverläufe mit limitiert-aufgebohrter Kompressionsmarknagelosteosynthese der Tibia und früher postoperativer Mobilisierung mit beschwerdeorientierter Belastung der verletzten Extremität werden dargestellt. Eine zusätzliche Stabilisierung der Fibulafraktur ist in der Regel nicht erforderlich. Die Zeitabläufe der Rehabilitationsmaßnahmen und die Wiederaufnahme des Trainings werden geschildert. Zwei Athletinnen konnten in den Wettkampfsport zurückkehren, nur eine konnte ihr Leistungsniveau im Weltcup wieder erreichen. Auf spezielle Weichteilprobleme, mögliche Fehlerquellen im Behandlungsverlauf und den kritischen Zeitpunkt der Metallentfernung wird eingegangen. Mit einem optimierten Behandlungskonzept mit belastbarer Osteosynthese und verzahnter früher postoperativer Rehabilitation ist eine Fortsetzung der Karriere des alpinen Skiprofis möglich. Fractures of the lower leg due to skiing accidents remain an important concern. Few studies have focussed on the special demands of professional athletes who sustain these injuries. We present our experience with three cases of lower leg fractures in competitive professional downhill skiers and discuss management and treatment concepts. We performed limited reamed compression nailing in all the patients presented because it offers the advantages of high mechanical stability and optimized fragment apposition. Plate osteosynthesis of the fibula is not required in most typical fractures. All patients resumed ski training. Two of them returned to World Cup. Only one achieved her pre-injury World Cup level of performance and success. In conclusion, a successful return for professional skiers with lower leg fractures is feasible using an optimized treatment strategy.
