Abstract

Bei einer Kniegelenksendoprothese wird ein künstlicher Oberflächenersatz einzelner Gelenkanteile oder des gesamten Gelenkes mittels Metall- und/oder Kunststoffimplantaten geschaffen. Der unikondyläre Kniegelenksersatz (sog. Schlittenprothese) stellt eine Sonderform der Gelenkprothese dar, bei der lediglich ein Teil des femoro-tibialen Gelenks (medial oder lateral) ersetzt wird. Mittel- bis endgradige Gonarthrosen nur eines Kompartiments (sog. Valgus- bzw. Varusgonarthrose) bei Patienten jenseits des 50. Lebensjahres stellen eine Indikation für den einseitigen Gelenkersatz dar. Die Insuffizienz eines Kreuzbandes ist eine Kontraindikation für den unikondylären Oberflächenersatz. Die klinischen Ergebnisse des unikondylären Kniegelenksersatzes sind bei korrekter Indikation und sorgfältiger Op-Technik vergleichbar mit dem trikompartimentellen Gelenkersatz bei geringerer Morbidität. Total joint replacement includes the resurfacing of the joint using metal or polyethylen implants. In unicondylar knee replacement only one part of the tibiofemoral joint (medial of lateral) is replaced. In patients over 50 years with osteoarthritis of the knee limited to one compartment a unicompartmental knee replacement may be indicated. Lesions of one of the cruciate ligaments are a contraindication for this approach. Good patient selection and a precise surgical technique have been shown to have clinical results comparable to those of tricompartmental joint replacement. The development of a minimal invasive surgical technique has led to the reduction of the morbidity of the procedure. In this article the medial unicompartmental replacement of the knee using a minimal invasive technique is described. Verf.-Referat
