Nach Ausschluss anderer Ursachen des vorderen Knieschmerzes kann mit einer in-traossären Druckmessung plus Schmerz-provokationstest bei einem Teil der Patien-ten die Symptomatik objektiviert und klas-sifiziert werden. Patienten mit einem erhöhten intraossären Druck (>25mmHg) und einem positiven Schmerzprovokationstest (=Hypertensionssyndrom der Patella) werden mit einer minimalinvasiven distalen Patelladekompressionsbohrung behandelt. Bei 27 Patienten mit therapieresistenten vorderen Knieschmerzen konnte eine Reduktion in der visuellen Analogskala von 7,6 auf 2,0 cm bei einem Follow-up von 3 Jahren dokumentiert werden. Abstract des Autors
Once other causes for anterior knee pain have been excluded, our patients with therapy resistant symptoms undergo a standardised diagnostic protocol including the measurement of the intraosseous pressure of the patella and a pain provocation test. We define the combination of a posi-tive provocation test and increased intraosseous pressure (>25mmHg) as a "hypertension syndrome of the patella". These patients with a symptomatic hypertension syndrome will then undergo a minimally invasive decompressive drilling of the distal patella. In a three year follow-up of 27 patients that underwent such a decompressive drilling of the patella, the evaluation utilising a visual analog scale revealed a pain reduction from an average of 7.6 to 2.0 cm.
