Abstract des Autors

To improve the differential diagnosis of occult intraosseous sports induced lesions of the knee we investigated the clinical findings of the bone bruise, mentioned first in 1988 by Yao and 1989 by Mink. In 11 patients (age 14-58 years) a bone bruise was diagnosed after sports induced injury of the knee by MRI. There were no clinical findings suggesting an injury of the ligaments or menisci. Plain radiographs showed no abnormalities. The common symptoms and course of the injuries were retrospectively listed according to the clinical records. In 5 cases bone bruise occured isolated. In 6 cases bone bruise was accompanied by a stable lesion of the ligaments. The characteristic symptom was long lasting pain for several month as well as the difficulty to find the correct diagnosis in absence of any clinical hint of capsular-ligamentary or meniscal lesions. For working out the correct diagnosis and especially for follow-up osseous percussion pain of the concerned area seems to be a valid aid. In sports induced knee injuries bone bruise should be taken into consideration if the examination of capsular and ligamentary stability and menisci as well as the plain radiographs are without pathological findings. Pain during loaded motion and percussion pain are the dominating clinical symptoms. Verf.-Referat
