A cause of sudden death during sport activity in a highly competitive cyclist is presented. Post-mortem examination of the heart demonstrated the anomalous origin of the right coronary artery from the left sinus of Valsalva and a transmural infarction involving the septum and the posterior wall. The athlete had suffered atypical symptoms. Hypo- and hyperkinetic arrhythmias were present at Holter monitoring but no cardiopathy was documented because the echocardiography and exercise test were normal. The case emphasizes that diagnostic protocols in sports medicine have to be extremely accurate to exclude even the rare cardiopathy. We discuss the sensibility of different methods to check congenital coronary anomalies. Verf.-Referat
