In the Department of Traumatology and Orthopaedic Surgery of the National Institute of Sports Medicine between 1987-1991 the substitution of LCA by arthroscopy was performed in 131 patients. The distribution was as follows: in 80 cases patella tendon, in 35 cases semitendinous-gracile tendon, in 4 cases semitendinous tendon, in 3 cases femoral fascia graft and in 9 cases plastic ligament were implanted. The mean age of the operated patients (113 men, 18 women) was 24.8 years (15-43). The interruption of sports activity was 11 months in the average. Intraoperative complication (fracture of Kirschner-wire) occurred in three cases, postoperative one (thrombophlebitis) in one case. Infection did not occure. Seven patients had to be reoperated. Technical error occurred in 13% of the cases (17 patients). The technique of the LCA substitution is fully detailed. The attention is called to the importance of isometric LCA substitution, the indication and advantages of the closed operative technique is discussed. In the future longitudinal follow-up of numerous patients is necessary for the real valuation of the operative technique. Verf.-Referat
